Xylitol is a five-carbon sugar alcohol present in small amounts in fruits and vegetables and made commercially from plant biomass19. It works as a sugar substitute that lowers mutans streptococci counts and plaque rather than feeding them4, 6. Caries prevention is the main use, followed by nasal irrigation in chronic rhinosinusitis and otitis media prophylaxis in children. It is also used as a sweetener with a low glycemic response in diabetes20, 21. Typical users are children and adults at moderate-to-high caries risk and patients in postoperative sinus care.

A Cochrane review rated the evidence for xylitol products as low quality, with the clearest signal for fluoride toothpaste containing xylitol1. A review of gum trials found a caries-preventive effect that was absent for candies2. Nasal irrigation improved symptom scores but not endoscopic scores against saline14. A 2026 network meta-analysis of randomized trials after endoscopic sinus surgery found steroid irrigation more effective than xylitol solution on symptom scores22. Many products list xylitol, mostly as bulk powder and in combination products such as mints; see Form comparison.

Bloating, flatulence, and loose stool are the adverse effects reported in caries trials. A 2026 survey of Italian metabolic centres found dietary practice divided on whether xylitol is permitted in hereditary fructose intolerance23.
