Theobromine (3,7-dimethylpurine-2,6-dione) is a methylxanthine related to caffeine and present in high levels in cocoa5. Supplementation has been studied for mood and alertness; in healthy volunteers, low doses had limited effects while higher doses worsened mood and raised heart rate5. It is also tested for persistent cough and for enamel remineralization.

The evidence is mixed. In healthy adults, lower doses produced limited subjective effects, higher doses produced negative mood effects, and heart rate rose with dose5. A crossover trial found LDL cholesterol fell while postprandial glucose and insulin responses worsened. Blood pressure findings split: theobromine-enriched cocoa raised 24-hour ambulatory systolic pressure while lowering central systolic pressure6. A persistent cough trial missed its primary endpoint7. A 2024 meta-analysis of in vitro studies found theobromine increased enamel microhardness after demineralization, comparable to fluoride, without human data8. Products listing theobromine number 2577, mostly powders with some capsules; see Form comparison.

The cardiovascular signals explain the label cautions.4 Product labels warn against use, or ask for physician consultation, in people with high blood pressure, heart disease, or antidepressant use4.
