Specialized pro-resolving mediators (SPMs) are lipid mediators made from omega-3 fatty acids, including resolvins, protectins, and maresins, that promote the resolution of inflammation rather than blocking it2, 3. Supplements are used by adults with chronic pain and for oral inflammation such as periodontitis; use for recovery from exercise-induced muscle damage is proposed but has not been tested in human trials2.
Human research is limited to about seven small trials, in chronic pain, obesity, rheumatoid arthritis, knee osteoarthritis, and healthy volunteers. An open-label, non-randomized 2020 trial in adults with chronic pain found improved quality of life, pain intensity, and mood, with no change in blood inflammatory markers1. Support for muscle recovery rests mainly on preclinical studies2. A 2025 narrative review reported that commercial SPM-enriched marine oils contain monohydroxylated precursors and low-dose eicosapentaenoic acid and docosahexaenoic acid rather than active SPMs2. United States (US) label records list 1487 products listing specialized pro-resolving mediators, with enriched marine oil the form studied; see Form comparison.
Per the one US product label reviewed, the product contains fish (anchovy and herring), so source-material allergy is the main label precaution4.
