Sodium bicarbonate is an alkalinizing salt that raises blood bicarbonate and pH, acting as an extracellular buffer8. That buffering drives its main supplemental use before high-intensity exercise lasting roughly half a minute to twelve minutes. It is also used in chronic kidney disease (CKD) with metabolic acidosis and given intravenously for acidosis in critical illness.

The sports evidence is the most consistent: an umbrella review rated support for peak anaerobic power and muscular endurance as low to moderate quality. In critically ill adults with metabolic acidosis, a recent trial found no benefit on mortality or kidney events, though a meta-analysis found less renal replacement therapy5, 9. In soccer players, a 2025 randomized trial found oral and topical bicarbonate did not speed recovery from muscle damage, but oral use attenuated the later decline in sport-specific performance10. Many products list sodium bicarbonate, as powders and capsules; enteric-coated capsules produced fewer gastrointestinal symptoms in trained males11, and Form comparison covers the rest.

The main caution is sodium and alkali load: infusion trials reported metabolic alkalosis and hypernatremia12, 13. Some United States product labels say not to use it in pregnancy or nursing7.

  • SafetyWho should avoid it, who needs extra care, and the side effects reported in studies.
  • InteractionsThe medicines and supplements it interacts with, the effect of each, and what labeling says to watch.
  • Protocol notesThe doses used in studies, how and when they were taken, and how the forms on the market compare.
  • How it worksWhat it is and how it works in the body, in plain terms.
  • Published studiesThe studies behind each use, popular claims checked against them, and trials still under way.
  • Word of mouthWhat patients and practitioners report online, kept separate from the clinical evidence.
  • Clinical perspectiveA closing read of where the evidence is solid, where it is thin, and what has not been studied.
