Postbiotics, which include heat-killed probiotics and paraprobiotics, are defined by the International Scientific Association for Probiotics and Prebiotics (ISAPP) as a preparation of inanimate microorganisms and/or their components that confers a health benefit10. They act chiefly by fortifying the epithelial barrier and modulating immune responses11. Supplementation is used for bowel symptoms, with trials in chronic constipation, chronic diarrhea, and irritable bowel syndrome (IBS). It is also used as an adjunct during Helicobacter pylori eradication, antibiotic courses, and immune checkpoint inhibitor therapy in lung cancer.
A meta-analysis of randomized trials in IBS found lower symptom severity, with low certainty of evidence3. A systematic review of inflammatory conditions found heterogeneous preparations and risk of bias that require cautious interpretation12. In a 2024 cell-culture study, a fermented postbiotic reduced inflammatory markers in atopic dermatitis-like keratinocytes; no human trial has tested it. Products are capsules, softgels, and powders of heat-inactivated cells or fermentation metabolites, with 78 products listing postbiotics; see Form comparison.
Safety confirmation rests on limited in vivo research13. Three of 30 sampled US labels declare milk as an allergen9. Eleven carry wording asking patients to consult a practitioner first if they anticipate surgery or take medication9.
