Human milk oligosaccharides (HMOs) are carbohydrates abundant in breast milk that reach the colon and selectively feed bifidobacteria7. Supplementation aims to move the gut microbiota, stool characteristics, and intestinal immune markers of formula-fed infants toward those of breastfed infants8. Healthy adults also take 2'-fucosyllactose and lacto-N-neotetraose for a bifidogenic shift in the gut microbiota2.
Fewer parent-reported cases of bronchitis with 2'-fucosyllactose plus lacto-N-neotetraose were a secondary outcome that the authors say needs confirmation. Whether HMOs add clinical benefit over non-human oligosaccharides remains unknown9. In a 2025 study using vaginal cell cultures, tissue models, and a mouse model, HMOs with Lactobacillus crispatus reduced ascending Group B Streptococcus infection; no human trial has tested this. Many products list human milk oligosaccharides, and lacto-N-neotetraose is commercially added to infant formula10; see Form comparison.
Systematic review of manufactured HMO trials found supplementation consistently safe and well tolerated8. Per United States product labels, some supplements are marked not intended for children under 1 year, and one lists lactose6.
