Exogenous ketones are oral ketone salts, esters, or precursors such as 1,3-butanediol that raise blood beta-hydroxybutyrate without carbohydrate restriction7. The ketone serves as an alternative fuel and signaling molecule.1 Use follows from that effect: lowering blood glucose in type 2 diabetes1, cognitive support, recovery in athletes8, and heart failure physiology4.

A meta-analysis found ingestion lowers blood glucose, more so with monoesters than salts1. Another found modest cognitive gains over placebo2. Performance trials are mixed, mostly null9. A crossover trial in type 2 diabetes with heart failure with preserved ejection fraction found two weeks of oral ketone ester raised cardiac output and lowered filling pressures at rest and at peak exercise4. A 2026 randomized trial in military airborne trainees found ketone monoester gave limited protection of cognitive and balance measures after head strikes, without biomarker change10. Powders and capsules dominate, with 632 products listing exogenous ketones; see Form comparison.

Gastrointestinal symptoms and taste drove dropout in a prolonged-use trial in type 2 diabetes11.

  • SafetyWho should avoid it, who needs extra care, and the side effects reported in studies.
  • InteractionsThe medicines and supplements it interacts with, the effect of each, and what labeling says to watch.
  • Protocol notesThe doses used in studies, how and when they were taken, and how the forms on the market compare.
  • How it worksWhat it is and how it works in the body, in plain terms.
  • Published studiesThe studies behind each use, popular claims checked against them, and trials still under way.
  • Word of mouthWhat patients and practitioners report online, kept separate from the clinical evidence.
  • Clinical perspectiveA closing read of where the evidence is solid, where it is thin, and what has not been studied.
