Diosmin is a flavonoid glycoside obtained from citrus rinds, usually by conversion of hesperidin, and is given alone or as micronized purified flavonoid fraction (MPFF), which is mostly diosmin with a small hesperidin share. It acts as a venoactive compound, supporting venous tone, endothelial barrier integrity, and lymphatic drainage while limiting leukocyte adhesion and inflammatory mediator release9. People take it for leg symptoms and edema in chronic venous disease, for hemorrhoidal bleeding and pain, for pelvic congestion symptoms, and for swelling after knee arthroplasty. Typical patients are middle-aged adults, predominantly women, with heavy, swollen, or aching legs4.

A meta-analysis of MPFF in chronic venous disease found reduced ankle and calf circumference and less feeling of swelling, though heterogeneity among studies was high2. A randomized trial in total knee arthroplasty found less swelling and less pain during motion with diosmin, without more short-term complications6. Products come as non-micronized diosmin, MPFF, and diosmin-hesperidin combinations, and a review found no clinical advantage for micronization or added hesperidin on venous symptoms; see Form comparison10.
