Branched-chain amino acids (BCAAs) are the essential amino acids leucine, isoleucine, and valine, which supply muscle fuel and signal protein synthesis through the mechanistic target of rapamycin (mTOR) pathway10. Supplementation is used to support muscle recovery in athletes1, to address hepatic encephalopathy and muscle loss in cirrhosis2, and for sarcopenia in older adults, where the evidence is unclear.
In cirrhosis, a Cochrane review found low-certainty evidence that BCAAs reduce hepatic encephalopathy, with little to no effect on mortality2. In athletes, benefits on performance and body composition were negligible, though soreness after resistance exercise was attenuated1. The muscle protein synthesis response to BCAAs alone is smaller than the response to a complete protein11. A 2026 systematic review found reduced depressive symptoms with BCAA supplementation or higher dietary intake in several studies, with heterogeneous results6. Products are mostly capsules and powders; see Form comparison.
The reason behind the strictest caution is metabolic: in maple syrup urine disease, breakdown of these amino acids is impaired and leucine accumulates to toxic levels. Some United States product labels ask for physician consultation before use with diabetes or thyroid medication9.
