Alpha-GPC (L-alpha-glycerylphosphorylcholine), also called choline alfoscerate, is a choline-containing phospholipid metabolite that acts as a precursor of choline and acetylcholine, supporting cholinergic neurotransmission9. Supplementation is used for cognitive impairment from Alzheimer's disease, vascular dementia and cerebral ischemia9. Typical patients are older adults with cognitive decline from neurological or cerebrovascular conditions.

A meta-analysis found better cognition, function, and behavior with alpha-GPC, mostly combined with donepezil, in cerebrovascular-related impairment1. A separate meta-analysis favored it in vascular and post-traumatic mild cognitive impairment2. Several supporting trials are older open-label studies using injectable regimens10. Evidence in subthreshold depression is a 2026 open-label pilot. A 2026 Korean retrospective cohort found alpha-GPC use was not associated with kidney cancer risk, a question raised by its link to trimethylamine N-oxide11. Products are sold mainly as capsules, plus powders and liquids; see Form comparison.

Reviews note that possible risks of atherosclerosis and stroke await validation9. Health Canada labels ask for a practitioner's advice before use in pregnancy or breastfeeding12.
